What sertraline is & how it works
Sertraline is an oral prescription antidepressant, a selective serotonin reuptake inhibitor (SSRI) approved for depression and anxiety disorders. Doctors also prescribe it off-label to treat premature ejaculation (PE), the most common male sexual complaint, defined as ejaculation that consistently happens sooner than a man and his partner would like and causes distress.
It works through serotonin. The reflex that triggers ejaculation is regulated in part by serotonin in the nervous system: higher serotonin activity raises the threshold and lengthens the time to climax. Sertraline blocks the serotonin transporter, so more serotonin stays active at the nerve junction. On daily dosing this effect accumulates over the first weeks rather than working on the first dose.
It manages a symptom rather than curing a condition, and it works only while you take it. Sertraline is one option among several, and it is not first-line for everyone. Whether it suits you depends on how often the problem occurs, your general and mental health, and the other medicines you take. That is what the doctor's assessment is for.
PE and the ejaculatory reflex
Ejaculation is controlled by a serotonin-sensitive reflex. Lower serotonin signalling shortens the time to climax.¹

The SSRI blocks the serotonin transporter, so more serotonin stays active in the nerve synapse.²

Higher serotonin tone lifts the ejaculatory threshold, lengthening the time to ejaculation.

Getting sertraline in Thailand
Thai FDA status
Sertraline is registered with the Thai FDA as a prescription-only medicine for depression and anxiety. It is not approved anywhere for premature ejaculation, so any use for PE is off-label and must be doctor-directed. It is sold in Thailand under brands such as Zoloft alongside registered generics.
How to get it through Menscape

Safety vs the grey market
Antidepressants act on brain chemistry and interact with many other drugs, so they need medical oversight for dosing, interactions and safe stopping. Buying SSRIs online without a prescription is illegal in Thailand and carries a real counterfeit risk. Expats and visitors do not need Thai residency; bring your medical history and any current prescriptions.
Thai FDA warning. The regulator repeatedly warns against buying prescription medicines from unlicensed online sellers, where counterfeits are common and nobody is accountable for what you receive. Sertraline for PE is an off-label use decided by a physician.

Does it work? The evidence
SSRIs are the most-studied drug class for premature ejaculation. In the largest meta-analysis of controlled trials, daily SSRIs raised the intravaginal ejaculatory latency time (IELT), the measured time from penetration to ejaculation, several-fold. Daily sertraline produced roughly a fourfold geometric-mean increase, with paroxetine the most potent (around eightfold) and placebo close to 1.4-fold.
Two things to know before you start. On daily dosing the effect builds over 2–4 weeks rather than working on the first dose, and the benefit lasts only while you keep taking it. Response varies widely between men, and PE is best assessed alongside any coexisting erectile difficulty or performance anxiety, which a doctor will check for.

Side effects & who shouldn't take it
Common side effects
Nausea, headache, dry mouth, drowsiness or insomnia, sweating and reduced libido. Because it acts on serotonin, delayed orgasm is common: usually the intended effect in PE, but it can go further than wanted.
Serious but rare
Serotonin syndrome (when combined with other serotonergic drugs), abnormal bleeding and low blood sodium. Antidepressants carry a warning for increased suicidal thoughts in people under 25, so tell your doctor about any mood history.³
Not suitable for
Anyone taking a monoamine oxidase inhibitor (MAOI) or pimozide, men with a history of bipolar disorder or seizures, significant liver disease, or a known hypersensitivity to sertraline. It is not used for PE under age 18.
Interactions & stopping
Combining it with tramadol, triptans, other SSRIs or St John's wort raises serotonin-syndrome risk; NSAIDs and blood thinners raise bleeding risk. Do not stop abruptly. Taper with your doctor to avoid discontinuation symptoms.

Frequently asked questions
Can I buy sertraline over the counter in Thailand?
No. It is a prescription-only medicine that must be prescribed by a doctor. Antidepressants are not sold over the counter, and buying them online without medical oversight is illegal and carries a counterfeit risk.
Is sertraline approved for premature ejaculation?
No. No regulator has approved sertraline for PE; dapoxetine is the only SSRI licensed specifically for it. Sertraline is approved for depression and anxiety and is used off-label for PE, which is common and evidence-based but must be doctor-directed.
How long until it works?
Unlike an on-demand tablet, daily sertraline builds over time. Many men notice a change within 1–2 weeks, with the fuller effect over 2–4 weeks. It works only while you keep taking it.
How is it different from dapoxetine?
Dapoxetine is a short-acting SSRI made specifically for PE and taken 1–3 hours before sex. Sertraline is longer-acting and usually taken daily. A doctor helps choose based on how often you have sex, your health and your other medicines.
Will it affect my mood or libido?
It can, because it is an antidepressant. Reduced libido and delayed orgasm are the most relevant effects for PE. Tell your doctor if the effect feels too strong or if your mood changes, so the plan can be adjusted.
Can I take it only when I need it?
Some doctors use on-demand sertraline a few hours before sex, but daily dosing is better studied for PE. Which approach suits you is a medical decision your doctor makes with you.
Can I take it if I also have erectile difficulty?
Sometimes. When ED and PE occur together, a doctor may combine an SSRI with a PDE5 inhibitor, but this needs supervision because of interactions. Your assessment will check for both.
What happens if I stop taking it?
The benefit fades once the drug clears your system, and stopping suddenly can cause discontinuation symptoms such as dizziness and irritability. Always taper with your doctor rather than stopping abruptly.
