What dapoxetine is & how it works

Dapoxetine is an oral prescription medicine used to treat premature ejaculation (PE), one of the most common male sexual health conditions. Unlike other treatments borrowed from psychiatry, it was designed from the start for PE, and it is taken only when needed rather than every day.

Ejaculation is a reflex controlled partly by serotonin signalling in the brain and spinal cord. In men with PE that reflex triggers quickly. Dapoxetine is a short-acting selective serotonin reuptake inhibitor (SSRI): it briefly raises serotonin at nerve junctions, which raises the threshold for the reflex and lengthens the time to ejaculation.

What makes it different from everyday antidepressants is its speed. It is absorbed within 1–2 hours and cleared quickly, so it can be taken 1–3 hours before sex and does not build up in the body the way a daily SSRI does. It manages the symptom on the day it is taken; it is not a cure, and a proper assessment matters because PE can have several causes.

    Ejaculation is a spinal reflex modulated by serotonin signalling in the brain and spinal cord.

    In men with premature ejaculation the reflex is triggered quickly, often within a minute.¹

    A higher threshold lengthens time to ejaculation, then the drug clears quickly.²

Getting dapoxetine in Thailand

Thai FDA status

Registered with the Thai FDA and prescription-only, in 30 mg and 60 mg tablets. It is sold in Thailand under brands such as Priligy. Notably, dapoxetine has never been approved in the United States, so many American expats first come across it here.³

Where it's legal to get

From a doctor at a licensed clinic, or dispensed by a licensed pharmacy on a prescription. A teleconsultation with licensed telepharmacy delivery is also legal in Thailand.⁴

For expats & visitors

No Thai residency is required. If you already take dapoxetine abroad, bring your details and a doctor can review continuity of treatment before dispensing.

Thai FDA warning. The regulator repeatedly warns against buying prescription medicines from unlicensed online sellers. Counterfeit sexual health products are common, and nobody is accountable for what you receive.⁵

Does it work? The evidence

Dapoxetine is the most-studied medicine for premature ejaculation. In pooled phase III randomised, placebo-controlled trials, the average time from penetration to ejaculation increased from under one minute at baseline to about 2.8 minutes on the 30 mg dose and 3.3 minutes on 60 mg, compared with 1.8 minutes on placebo.²

Two things to keep realistic. It does not work for every man, and it is not a cure: it lengthens time to ejaculation on the days you take it, and the effect fades as the drug clears. Many men also improve with behavioural techniques, and the strongest results often come from combining medicine with those techniques rather than relying on either alone.

Side effects & who shouldn't take it

Common side effects

Nausea, dizziness, headache, diarrhoea and trouble sleeping are the most common, are usually mild, and tend to ease with the lower dose. Taking the tablet with a full glass of water and avoiding alcohol reduces them.⁶

Fainting & low blood pressure

Dapoxetine can cause a sharp drop in blood pressure, dizziness or fainting (syncope), most often with the first dose or when mixed with alcohol. If you feel light-headed, lie down and raise your legs, and tell your doctor about any history of fainting.

Not suitable for

Men under 18 or over 64, women, and men with significant heart disease (heart failure, rhythm or conduction problems, ischaemic heart disease) or moderate-to-severe liver disease. Caution too with a history of depression, mania or bipolar disorder.

Dangerous combinations

Never combine it with MAOI antidepressants (or within 14 days of them), thioridazine, other SSRIs or SNRIs, lithium, tramadol or St John's Wort, because of serotonin syndrome risk. Tell your doctor every medicine and supplement you take.

Creams, sprays or wipes applied before sex that briefly reduce sensitivity. They can be used on their own or, in some cases, alongside an oral medicine when a doctor advises.

Oral · daily, off-label

Daily SSRIs

Antidepressants such as paroxetine or sertraline, taken every day, also lengthen time to ejaculation. They act more slowly and are used off-label, so a doctor may discuss them if on-demand treatment is not enough.

When ED coexists

Combined treatment

Some men have both premature ejaculation and erectile difficulty. Treating the erectile problem can improve control, and a doctor decides whether combining treatments is safe, since both can lower blood pressure.

Can I buy dapoxetine over the counter in Thailand?

No. It is a prescription-only medicine: a doctor must prescribe it and it is dispensed through a licensed pharmacy. Products sold without that oversight are illegal and carry a real counterfeit risk.

How is dapoxetine different from a regular antidepressant?

Can I take it with a medicine for erectile dysfunction?

Sometimes, if you have both conditions, but only under a doctor's guidance, because both can lower blood pressure. Never combine prescription medicines without medical advice.

Is a teleconsultation enough, or do I need to visit the clinic?

For many men a teleconsultation is sufficient and legal in Thailand. Your doctor will ask you to come in to Asoke if an in-person check, such as blood pressure or a heart review, is needed.

Will it cure premature ejaculation?

No. It manages the symptom on the days you take it and does not cure the underlying condition. Pairing it with behavioural techniques often gives the best long-term result.
