What TRT is & what it isn't
Testosterone replacement therapy (TRT) restores testosterone to a healthy range in men whose bodies no longer make enough of it, a condition called hypogonadism. Testosterone supports libido, erections, muscle and bone strength, red blood cell production, mood and energy, so when it runs genuinely low, several of those systems can suffer at once.
TRT is not a dose stacked on top of a normal level. It replaces what is missing, bringing a deficient man back into the normal range and no higher. That is why diagnosis matters: it treats a measured shortfall, not a wish to feel sharper when your levels are already fine.
At Menscape we use two well-established forms: a transdermal gel applied to the skin daily, and long-acting intramuscular injections given on a schedule. They differ in convenience and how steady they keep your levels, which the doctor talks through with you.
Above all, TRT is not a fix for tiredness on its own. Fatigue, low mood and low drive have many causes, from poor sleep and stress to thyroid problems and depression. Without two low morning blood tests to back it up, testosterone is the wrong tool, and taking it when you do not need it carries real risks.
Testosterone's job
It supports libido, erections, muscle and bone, red blood cells, mood and energy. Low levels can affect several at once.
Replace, not boost
TRT brings a deficient man back into the normal range, not above it. It corrects a measured deficiency.
Gel or injection
A daily skin gel or a scheduled intramuscular injection. The doctor helps you choose based on your levels and routine.
Not a tiredness cure
Fatigue alone is not a reason to start. Without low morning labs and matching symptoms, testosterone is the wrong tool.

Getting TRT in Thailand
Prescription only
Testosterone is a prescription-controlled medicine in Thailand, dispensed through licensed clinics and pharmacies. It is not sold over the counter, and sourcing it without a doctor is both illegal and unmonitored.
How you actually qualify
The bar is two separate morning blood tests showing total testosterone below roughly 300 ng/dL, together with symptoms that fit. One borderline reading on its own is not enough, which is exactly why we repeat the test.
The gym and grey market
Bangkok gyms and online sellers offer testosterone and blends with no diagnosis and no follow-up, often at doses well above replacement. Supervised TRT means the right dose, regular blood checks and someone accountable if something changes.
Bring your existing labs. If you already have testosterone results from home or another clinic, bring them. We will still retest properly, with a repeat morning sample, before starting or continuing treatment.

What the evidence shows
In men with genuinely low testosterone, replacement usually improves sexual desire and, for many, erectile function, alongside modest gains in mood, energy and bone density. The Testosterone Trials, a set of placebo-controlled studies in men over 65 with low levels, found consistent improvements in sexual activity and mood over 12 months. Libido tends to respond first, within 3 to 6 weeks, while effects on body composition and bone build over months.
On the safety question men ask most, the TRAVERSE trial randomised over 5,000 middle-aged and older men with low testosterone and existing heart-disease risk. Over about three years, major cardiac events occurred in 7.0% on testosterone versus 7.3% on placebo, no meaningful difference. It did, however, record more atrial fibrillation and blood clots, which is part of why monitoring matters.
The evidence is strongest in men with a clear, confirmed deficiency. In men whose levels are already normal, the same benefits do not appear and the risks remain. That is why we treat a diagnosis, not a number you would like to see move.

TRAVERSE was a randomised, placebo-controlled trial of over 5,000 men with hypogonadism and cardiovascular risk (Lincoff et al., NEJM 2023). Individual results vary.

Risks & monitoring
Thicker blood
Testosterone raises red blood cell production, and in some men the blood gets too thick (raised haematocrit), which can increase clot risk. We check it at baseline, at 3 to 6 months, then yearly, and adjust or pause if it climbs past about 54%.
Prostate and PSA
TRT does not appear to cause prostate cancer, but it can stimulate one that is already present. We check PSA and prostate health before starting and during treatment, and involve a specialist if anything looks off.
Fertility
Gel or injected testosterone shuts down the body's own signal to the testes and can reduce sperm production, sometimes lastingly. Men who want children may be better suited to enclomiphene or hCG, which raise testosterone while preserving fertility.
Skin and mood
Acne, oilier skin and occasional irritability can occur, usually early and dose-related. Tell your doctor if mood changes are significant, and we adjust the dose or the form.

Enclomiphene
A tablet that prompts your own testes to make more testosterone by nudging the pituitary, rather than replacing the hormone from outside. It keeps fertility intact, which makes it a common first choice for younger men.
For men wanting children
Clomiphene & hCG
Clomiphene works much like enclomiphene, while hCG mimics the pituitary signal directly. Both raise testosterone while supporting sperm production, and are often used when fertility is the priority.
Genuine, foundational levers
Sleep, weight & strength
Poor sleep, excess body fat and inactivity all lower testosterone. Fixing sleep, losing visceral fat and resistance training can meaningfully raise it, and are worth addressing before or alongside any medication.

Can I buy testosterone over the counter in Thailand?
No. It is a prescription-controlled medicine that must come from a doctor and a licensed pharmacy. Testosterone sold through gyms or online sellers carries no diagnosis, no correct dosing and no monitoring, and is often counterfeit.
How do I know if my testosterone is actually low?
Low testosterone is diagnosed on two separate morning blood tests showing a total level below roughly 300 ng/dL, together with symptoms such as low libido, fatigue and low mood. A single borderline result is not enough.
Is TRT safe for the heart?

How soon will I feel a difference?
Libido often responds within 3 to 6 weeks. Energy and mood tend to follow over the first few months, while changes in muscle, fat and bone build more slowly. We review progress with repeat bloods at 3 to 6 months.
Do I have to stay on TRT for life?
It depends on the cause. Some men have a permanent deficiency and stay on treatment long term; others are treated for a reversible cause. If you stop, testosterone returns to your baseline and symptoms usually come back.
I already take TRT abroad. Can I continue it in Bangkok?
Usually, yes. Bring your prescription and recent labs. A local doctor will retest properly with a morning sample, review your protocol and monitoring, and continue treatment if it remains appropriate.
