What dutasteride is & how it works

Dutasteride is an oral prescription medicine. It was first developed to shrink an enlarged prostate (benign prostatic hyperplasia), and it lowers the same hormone that drives male-pattern hair loss, which is why doctors also use it off-label for the scalp.

Male-pattern hair loss is driven by dihydrotestosterone (DHT), a by-product of testosterone that slowly shrinks genetically sensitive follicles at the temples and crown. Two enzymes, type 1 and type 2 5α-reductase, make DHT. Finasteride mainly blocks type 2; dutasteride blocks both. That dual action lowers serum DHT by roughly 90%, compared with about 70% for finasteride.

Blocking DHT more deeply is not automatically better for everyone, and dutasteride is a stronger, longer-acting drug with less long-term hair data than finasteride. Whether it is the right choice depends on your pattern of loss, your response to earlier treatment, your health history and your goals. That is what the doctor's assessment is for.

    Two enzymes, type 1 and type 2 5α-reductase, convert testosterone into DHT.

Getting dutasteride in Thailand

Thai FDA status

Registered with the Thai FDA for benign prostatic hyperplasia (enlarged prostate) and prescription only. It is sold in Thailand under brands such as Avodart. There is no registered hair-loss indication here, so for hair it is prescribed off-label, a legal decision made by a doctor.⁵

How to get it through Menscape

Skip the grey market

Because it is not stocked casually, some men turn to unlicensed online sellers. That is where counterfeits and wrong doses appear, and nobody is accountable for what arrives. A drug that suppresses DHT for weeks is not something to self-source without a doctor.

Thai FDA warning. The regulator has repeatedly warned against buying prescription medicines from unlicensed online sellers, where counterfeit and substandard products are common.⁶

Does it work? The evidence

Dutasteride's hair-loss use rests on a smaller evidence base than finasteride, but the studies are consistent. In a randomised, active- and placebo-controlled trial comparing dutasteride 0.5 mg, finasteride 1 mg and placebo over 24 weeks, dutasteride increased hair count more than finasteride and clearly beat placebo. An earlier Korean phase III study reached the same conclusion for dutasteride 0.5 mg in men with androgenetic alopecia.²³

The mechanism explains the edge: dutasteride lowers serum DHT by roughly 90% versus about 70% for finasteride 1 mg. A network meta-analysis of 5α-reductase inhibitors ranked dutasteride 0.5 mg among the most effective oral options for hair count.⁷ Two caveats stand. Long-term hair data is thinner than finasteride's, and for hair the use is off-label. Results still take time: shedding settles over 3 to 6 months and regrowth is judged at 12.

Side effects & who shouldn't take it

Sexual side effects (uncommon)

Decreased libido, erectile difficulty, reduced ejaculate volume, and breast tenderness or enlargement (gynaecomastia). Rates are low, in the low single digits, and broadly similar to finasteride. Most resolve on stopping, though that can take longer with dutasteride.¹

Because it lingers

Dutasteride has a long half-life of around 5 weeks, so it stays active well after your last capsule. Any side effects can take longer to clear, and you must not donate blood while taking it or for 6 months after stopping.¹

Not suitable for

Women, especially those pregnant or of childbearing potential, must not take it or handle leaking capsules, as it is teratogenic to a male fetus. Also unsuitable for men under 18, anyone with hypersensitivity to it, or significant liver disease.

Tell your doctor

Dutasteride roughly halves PSA, so tell any doctor testing your prostate that you take it. Mood changes have been reported. Because hair use is off-label, honest reporting and follow-up matter more, not less.

Can I buy dutasteride over the counter in Thailand?

No. It is a prescription-only medicine, registered for the prostate rather than hair, and for hair loss it is used off-label, so a doctor must prescribe it. Anything sold without that oversight is illegal and a counterfeit risk.

Is dutasteride approved for hair loss?

Not in Thailand or the United States. It is approved for male hair loss in a few countries such as South Korea and Japan, and everywhere else doctors use it off-label. Its registered use in Thailand is for enlarged prostate.

Is dutasteride stronger than finasteride?

It blocks both DHT enzymes rather than mainly one, lowering serum DHT by around 90% versus about 70%, and a head-to-head trial showed more hair growth over 24 weeks. But finasteride is more studied and remains first-line, so stronger is not automatically the right choice for you.
