Topical finasteride plus minoxidil is a compounded prescription medicine applied to the scalp to treat male-pattern hair loss (androgenetic alopecia), the gradual, hormone-driven thinning at the temples and crown that affects most men at some point. The clinic's version combines minoxidil 5% with finasteride in a single solution.

It attacks hair loss from two directions at once. Minoxidil is a vasodilator that improves blood flow to the follicle and extends the hair's growth (anagen) phase, stimulating follicles directly. Finasteride blocks the enzyme 5α-reductase, lowering levels of dihydrotestosterone (DHT), the hormone that progressively shrinks genetically sensitive follicles. One works on growth, the other on the cause.

The reason to apply finasteride to the scalp rather than swallow it is exposure. The topical route concentrates the drug where it is needed while limiting how much enters the bloodstream, which is why many men consider it when they are worried about the side effects linked to oral finasteride. It is a compounded preparation, so a doctor decides whether it is a sensible choice for you rather than an oral option.

Getting topical finasteride + minoxidil in Thailand

Thai FDA status

Minoxidil topical is sold over the counter in Thailand (under brands such as Nuhair), but finasteride is classified as a dangerous drug (ยาอันตราย). A product that combines the two therefore contains a prescription ingredient and is prescription-only.⁴

Does it work? The evidence

Both ingredients are well evidenced on their own. Oral finasteride is among the most-studied treatments in dermatology, and topical minoxidil is one of only two treatments with decades of randomised-trial support. A 2022 phase III trial of topical finasteride found it improved hair count comparably to the oral tablet while reducing DHT in the bloodstream far less, which is the rationale for using it on the scalp.³

Combining them aims to pair minoxidil's follicle stimulation with finasteride's DHT blockade in one step. The evidence for compounded combinations specifically is smaller and shorter-term than for the individual oral drugs, which is why a doctor weighs it against oral options rather than assuming it is better for everyone. Results also take time: shedding stabilises over 3–6 months and hair count is assessed at 12, and the benefit continues only while you keep using it.

Side effects & who shouldn't use it

Common, usually mild

Scalp irritation, itching, dryness or flaking, and an initial increase in shedding in the first weeks as follicles cycle. Contact dermatitis can occur, sometimes as a reaction to the alcohol or propylene glycol base rather than the active drugs.

Finasteride-related effects

Because some finasteride is absorbed, the sexual side effects linked to the oral tablet, such as reduced libido or erectile difficulty, remain possible even though systemic exposure is lower with the topical. Tell your doctor about any persistent change.

Not suitable for

Women who are pregnant, breastfeeding or may become pregnant must not use or handle it, as finasteride can harm a developing male fetus. It is also not for under-18s or anyone allergic to either ingredient. Let it dry fully and wash your hands to avoid transfer to partners or children.

Tell your doctor if

You have a heart condition (minoxidil can rarely raise heart rate through absorption), a history of mood changes, scalp wounds or dermatitis, or you are trying to conceive. Finasteride lowers PSA, so mention it before any prostate blood test.
