What minoxidil is & how it works
Minoxidil is a topical medicine used to treat male-pattern hair loss (androgenetic alopecia), the gradual, hormone-driven thinning at the temples and crown that affects most men at some point. It is the most-studied topical treatment for the condition.
It started life as an oral blood-pressure drug in the 1970s. Doctors noticed patients growing thicker hair, and a topical version was developed and approved for hair loss by the US FDA in 1988. Applied to the scalp, minoxidil widens local blood vessels and extends the growth (anagen) phase of the hair cycle, so more follicles produce longer, thicker hairs.
It stimulates growth rather than blocking the hormone that drives the loss. That is why it is often combined with finasteride, which works on the hormonal side, and why it only works for as long as you keep using it.
Minoxidil is one option among several. Whether the topical solution, a compounded combination, or low-dose oral minoxidil suits you depends on your pattern of loss, your scalp and your health history. That is what a pharmacist or doctor check is for.
Originally a blood-pressure drug
Oral minoxidil was made to lower blood pressure; thicker hair growth turned up as a side effect.
Activated in the scalp
Applied to skin, it is converted by sulfotransferase enzymes into minoxidil sulfate, the active form.¹
Follicles stay in growth phase
It widens blood vessels and lengthens the anagen phase, so more follicles grow longer, thicker hair.⁷
Shedding, then regrowth
An early jump in shedding is normal as follicles reset; visible thickening follows over 3–6 months.

Getting minoxidil in Thailand
Thai FDA status
The topical 5% solution is registered with the Thai FDA and sold over the counter at licensed pharmacies, with no prescription needed. It is sold in Thailand under brands such as Rogaine, alongside registered generics.⁴
Where to get it
Because the topical is OTC, a pharmacist can supply it directly after a quick check. Low-dose oral minoxidil is different: it is a prescription medicine, so a doctor must assess you before it can be dispensed.
For expats & visitors
No Thai residency is required to buy the topical solution. If you already use minoxidil abroad, a pharmacist or doctor can review continuity of your routine and the right pack size.
Thai FDA warning. The regulator warns against buying medicines from unlicensed online sellers. Counterfeit and mislabeled minoxidil with the wrong strength is common, and nobody is accountable for what you receive.⁴

Does it work? The evidence
Topical minoxidil is one of only two treatments with strong evidence for male-pattern hair loss, the other being finasteride. In a randomised trial comparing strengths in men, the 5% solution produced about 45% more hair regrowth than the 2% solution at 48 weeks, with a faster onset.² Most men who respond see thicker, denser coverage rather than a fully restored hairline.

Results take time: an increase in shedding in the first weeks is normal as follicles reset, with visible thickening over 3 to 6 months and a full assessment at 12. And the effect lasts only while you use it, because follicles return to their previous cycle over the months after you stop.

Side effects & who shouldn't use it
Common side effects
Scalp irritation, itching, dryness or flaking, and contact dermatitis, often from the propylene glycol carrier. An initial increase in shedding in the first few weeks is expected and settles as new hair comes through.
Less common, from absorption
If overused or in sensitive people, small amounts can be absorbed, causing unwanted facial or body hair, a faster heartbeat, dizziness, or fluid retention and ankle swelling. Stop and see a doctor if these occur.
Not suitable for, or check first
People with a heart condition, low blood pressure or on other blood-pressure medicines, pregnant or breastfeeding women, anyone under 18, and those allergic to minoxidil or propylene glycol should speak to a doctor before use.
Oral minoxidil needs supervision
Low-dose oral minoxidil for hair is used off-label and must be doctor-monitored. As a blood-pressure drug it can affect heart rate, blood pressure and fluid balance, so it is never a DIY option.³

Can I buy minoxidil over the counter in Thailand?
Yes, for the topical form. The 5% solution is registered with the Thai FDA and sold over the counter at licensed pharmacies without a prescription. Low-dose oral minoxidil is different: it is a prescription medicine, so a doctor must assess you first.
How long until minoxidil works?
Give it time. An early increase in shedding in the first weeks is normal, visible thickening comes over 3 to 6 months, and a full assessment is made at 12 months. Consistent daily use matters more than anything.
Do I have to use minoxidil forever?
The effect lasts only while you use it. If you stop, follicles gradually return to their previous cycle over the following months and the gains fade, so most men keep using it to hold results.
Is the initial shedding normal?
Yes. A temporary increase in shedding in the first two to eight weeks is expected as follicles reset into a new growth phase. It usually settles, and it is not a sign the treatment is failing.
What is the difference between 5% and 2% minoxidil?
In men, the 5% solution is more effective and works faster; in one controlled trial it gave about 45% more regrowth than 2% at 48 weeks. The 2% strength is the older concentration and can cause less scalp irritation for some people.
Can I use minoxidil and finasteride together?
Yes. They work on different mechanisms, so they are commonly combined for a stronger effect. A doctor can advise on oral finasteride or a compounded topical finasteride and minoxidil solution.
Is oral minoxidil safer or stronger than the topical?
Low-dose oral minoxidil is used off-label and can help men who find the topical messy or irritating. It is not automatically safer: as a blood-pressure drug it can affect heart rate and fluid balance, so it is prescription only and needs monitoring.
Is minoxidil safe if I have a heart condition?
Check with a doctor first. Minoxidil is a vasodilator, and small amounts of the topical can be absorbed, so anyone with heart disease, low blood pressure or on blood-pressure medicines should be assessed before starting. Oral minoxidil in particular needs medical supervision.
