Asymptomatic hypospadias in an adult man doesn’t require repair. Indications for adult repair: bothersome urinary stream (spraying, downward direction), recurrent urinary infections, sexual dysfunction due to chordee, cosmetic concerns affecting self-confidence, or complications from prior childhood repair.
What’s the success rate?
Single-stage repair for distal adult hypospadias: 75-90% success. Two-stage Bracka repair for proximal or revision cases: 70-85% success. Success defined as a straight stream from the tip without fistula or stricture, and acceptable cosmetic outcome.
What are the risks?
Most common: urethral fistula (new opening) and urethral stricture · both can be managed with secondary repair. Less common: graft loss, persistent chordee, cosmetic dissatisfaction. We discuss all risks at consultation.
How long is recovery?
Catheter for 7-14 days. Light activity at 2 weeks. Return to work 2-3 weeks for desk work, longer for physical work. Sexual activity restricted for 6 weeks.
