What tadalafil is & how it works
Tadalafil is an oral prescription medicine used to treat erectile dysfunction (ED), the persistent difficulty getting or keeping an erection firm enough for sex. ED is a genuine medical condition, and it is often an early warning sign of a circulation or heart problem, which is one reason a proper assessment matters.
It works on the blood flow that produces an erection. When a man is sexually aroused, nerves release nitric oxide, which raises a signalling molecule called cGMP that relaxes the blood vessels in the penis so they fill with blood. An enzyme, PDE5, breaks cGMP down. Tadalafil blocks PDE5, so cGMP lasts longer and a natural erection becomes easier to achieve and maintain.
Two things follow from that. First, tadalafil does not create an erection on its own; sexual stimulation is still needed. Second, it is much longer-acting than other tablets in its class, with a half-life of about 17.5 hours and an effect that can last up to 36 hours, which is why it can be taken either on demand or as a small daily dose. The same daily dose also relaxes the muscle of the prostate and bladder neck, easing the urinary symptoms of benign prostatic enlargement (BPH). Whether it is right for you depends on your health history. That is what the doctor's assessment is for.
Arousal releases nitric oxide
Sexual stimulation triggers nitric oxide, which raises cGMP inside the penis.

Getting tadalafil in Thailand
Thai FDA status
Registered with the Thai FDA and prescription-controlled. The original brand product and several registered generics are sold in Thailand, in daily-use and on-demand strengths, through licensed channels.⁵
Where it's legal to get
From a licensed clinic where a doctor prescribes it, or a licensed pharmacy that dispenses it. Teleconsultation with licensed pharmacy delivery is also legal in Thailand.⁷

No Thai residency is required. Bring your medical history and a list of current medicines, especially any heart tablets, so the doctor can check tadalafil is safe to combine with them.
Thai FDA warning. Erectile-dysfunction tablets are among the most counterfeited medicines sold online. The regulator has repeatedly warned against unlicensed online sellers, where fake or contaminated products are common and nobody is accountable for what you receive.⁶

Does it work? The evidence
Tadalafil is one of the best-studied ED treatments in urology. In integrated analyses of the randomised, placebo-controlled trials, about 81% of men taking tadalafil answered yes when asked whether treatment had improved their erections, compared with roughly 35% on placebo. Successful intercourse attempts and standardised erectile-function scores improved consistently across doses.²
Its defining feature is duration. A single dose stays effective for up to 36 hours, so it does not have to be timed tightly to activity, and a low daily dose keeps a steady level in the blood so no timing is needed at all.³ The same once-daily dose has separately been shown to reduce the urinary symptoms of an enlarged prostate.⁴ Results vary between men. Tadalafil treats the symptom; a doctor's assessment also looks for the underlying cause.

Side effects & who shouldn't take it
Common side effects
Headache, flushing, indigestion and nasal congestion are the most frequent. Back pain and muscle aches are more characteristic of tadalafil than other PDE5 inhibitors, often appearing 12–24 hours after a dose and easing on their own.¹
Rare but serious
Seek urgent care for an erection lasting more than 4 hours (priapism), sudden vision loss in one or both eyes, or sudden hearing loss. Stop and call a doctor if these occur.¹
Not suitable for
Anyone taking nitrate heart medicines or the pulmonary drug riociguat, because the combination can cause a dangerous drop in blood pressure. Also caution after a recent heart attack or stroke, with uncontrolled blood pressure, or severe liver disease.¹
Interactions to flag
Alpha-blockers for blood pressure or prostate symptoms, and some antifungals and HIV medicines, can interact. Tell the doctor every medicine and supplement you take, and go easy on alcohol, which worsens the blood-pressure effect.

Can I buy tadalafil over the counter in Thailand?
No. It is a prescription-controlled medicine that must be prescribed by a doctor or dispensed by a licensed pharmacist. Long-acting ED tablets sold openly online or in unlicensed shops are frequently counterfeit and unsafe.
How is tadalafil different from sildenafil?
Both are PDE5 inhibitors, but tadalafil lasts far longer: up to 36 hours from one dose, versus around 4–6 hours for sildenafil. That longer window is why tadalafil can be taken on demand or as a small daily tablet.
How long does it take to work, and how long does it last?
On-demand, it typically starts working within 30 minutes to 2 hours and can remain effective for up to 36 hours. A low daily dose keeps a steady level in your blood so timing isn't needed.
Do I still need to be aroused for it to work?

Does tadalafil really help an enlarged prostate?

I already take tadalafil from abroad — can I continue it in Thailand?
