Prostate Biopsy · TRUS-Guided and MRI-Fusion in Bangkok
Prostate biopsy is the only way to definitively diagnose prostate cancer. We offer both classic TRUS-guided biopsy (transrectal ultrasound systematic sampling) and modern MRI-fusion biopsy (where pre-biopsy MRI lesions are precisely targeted during the biopsy itself). MRI-fusion gives higher cancer detection rates and lower rates of clinically insignificant findings.

How to Prepare for Prostate Biopsy
Pre-biopsy MRI (if MRI-fusion planned)
Multiparametric prostate MRI 1-4 weeks before biopsy to identify suspicious lesions.Antibiotic prophylaxis
Targeted antibiotics based on rectal swab culture · reduces sepsis risk after transrectal biopsy.Stop blood thinners
5-7 days before biopsy only if cleared by your prescribing doctor.Bowel preparation (transrectal only)
Enema before transrectal biopsy to clear stool.Fast pre-procedure if TIVA
No food or water for 6 hours if you’re having sedation.Plan for transport home
You cannot drive yourself home after sedation.

What Happens During Prostate Biopsy
Anesthesia
Local anesthesia (transrectal) or TIVA / spinal (transperineal).Imaging guidance
Transrectal ultrasound probe (TRUS) or fusion of pre-biopsy MRI with real-time ultrasound (MRI-fusion).Targeted cores
For MRI-fusion: 2-4 cores from each MRI-visible lesion.Systematic cores
10-12 cores sampled across both sides of the prostate to ensure comprehensive coverage.Tissue handling
All cores are labeled by location and sent for pathology with detailed mapping.Discharge
Same day. Mild blood in urine, semen, or stool for 1-3 weeks is normal.

About Prostate Biopsy
MRI-Fusion Biopsy Available
Not all Bangkok urology centers offer MRI-fusion · we use modern fusion platforms for precision-targeted sampling of MRI-visible lesions.
Transperineal Option
For lower infection risk and better access to anterior prostate lesions, transperineal biopsy is offered as an alternative to transrectal.
Pre-Biopsy MRI Interpretation
Our radiology team uses PI-RADS standardized scoring · clear lesion mapping before biopsy reduces unnecessary cores.
Private, Discreet Recovery
Confidential care, WhatsApp recovery support, English-language pathology reports.
Frequently asked questions
Should I have TRUS or MRI-fusion biopsy?
MRI-fusion is preferred if you have an elevated PSA without prior biopsy (or a negative prior biopsy with persistently elevated PSA) and your MRI shows suspicious lesions. TRUS-guided systematic biopsy is appropriate when MRI is unremarkable but biopsy is still indicated based on PSA trajectory.
Is the biopsy painful?
Discomfort more than pain. Transrectal biopsy is done under local anesthesia (lidocaine block) and feels like rubber-band snaps. Transperineal is done under TIVA or spinal and is painless during the procedure · mild perineal soreness for a day or two afterward.
What are the risks?
Most common: blood in urine (up to 6 weeks), blood in semen (up to 6 weeks), blood in stool after transrectal (resolves in days). Less common: urinary infection (1-3%, lower with prophylactic antibiotics), sepsis (rare, lower with transperineal approach), urinary retention.
How long until I get results?
Pathology results typically take 5-10 business days. Your urologist reviews results with you in person or via secure follow-up consultation.
How much does prostate biopsy cost in Bangkok?
Cost varies significantly between TRUS-guided and MRI-fusion biopsy, and depends on anesthesia choice (LA, TIVA, spinal, GA). Pre-biopsy MRI is a separate cost. Book a consultation for a personalized quote.
