What surgery can and cannot change
Start with the baseline, because most men who judge themselves as small are measuring against a distorted reference point. A pooled analysis of measurements from more than 15,000 men put average erect length at about 13.1 cm (5.2 inches) and average erect girth at about 11.7 cm (4.6 inches). A large share of men who ask about enlargement sit comfortably inside the normal range, which is exactly why a serious clinic screens expectations before it books an operating theatre.
Within that reality, here is what modern surgery can honestly offer:
Visible and flaccid length. Dividing the suspensory ligament lets the inner portion of the penis hang further outside the body. Published series report average gains of roughly 1-2 cm, and the final result depends heavily on how consistently you use a traction device afterwards. Erect length barely changes; what changes is how much of the shaft is visible at rest.
Girth. A soft silicone implant placed under the shaft skin, the class of device sold as Penuma and Himplant, adds circumference along the whole shaft. The largest published series reported a mean mid-shaft girth increase of 56.7%, measured on the flaccid penis, which in practice tends to mean roughly 4-6 cm of added flaccid circumference and a fuller appearance at rest. The proportional gain in the erect state is smaller, because the erect shaft starts from a larger baseline.
Correcting a physical cause. Men with a buried penis after weight gain, retraction after other pelvic surgery, or a prominent suprapubic fat pad often gain more from addressing that anatomy than from any enlargement technique on its own.
And what surgery cannot do: add several centimetres of erect length, guarantee a perfectly symmetrical result, or resolve distress that is really about body image rather than anatomy. A 2020 systematic review that included 1,192 men concerned about penis size, of whom 773 were treated and followed up, concluded that structured counselling should always come first, and that several widely marketed techniques have weak or no supporting evidence. We agree with that position, and we turn men away on that basis.

Three notes on reading this table. First, the savings are real but very uneven. Ligament release is mostly surgeon and theatre time, so Bangkok's lower operating costs cut the price by more than half. A branded silicone implant is different: the device itself is imported at a fixed dollar price, so Bangkok can only trim the hospital stay, anaesthesia team and surgical fees around it. The manufacturer of Himplant quotes most US patients $10,500-19,000 all-in, a range that overlaps almost entirely with Bangkok pricing, so on a like-for-like branded implant expect to save little or nothing, roughly 0-15%. The financial case for Bangkok rests on lengthening, combined plans and the surrounding cost of care, not on the implant itself.
Second, always ask what a quote includes. A meaningful package covers the surgeon, anaesthesiologist, theatre, the device where relevant, at least one night of observation if needed, medications, and scheduled follow-up visits. A suspiciously low headline price usually excludes half of that list.
Third, if a Bangkok clinic quotes a branded-implant procedure at a fraction of the device's known cost, something is wrong: either the implant is not what it claims to be or the surgical setting has been cut to the bone. We come back to this in the section on choosing a surgeon.
The procedures behind the prices
Suspensory ligament release
The suspensory ligament anchors the penis to the pubic bone. Dividing it, through a small incision hidden at the base of the shaft or in the pubic hairline, allows the portion of the penis that normally sits inside the body to drop forward and hang lower. The operation takes about an hour under general or spinal anaesthesia and is usually a day case.
The honest numbers matter here. In the most cited European series, 42 men underwent ligament division and gained a mean of about 1.3 cm of stretched length, and only around a third were satisfied with the result, largely because expectations had been set too high. With disciplined post-operative traction (a stretching device worn daily for several months, which stops the ligament re-attaching short), motivated patients can reach gains closer to 2 cm of visible length. Without traction, some men end up with no gain, and a few scar shorter than they started. One functional trade-off is worth knowing in advance: because the ligament also stabilises the erect penis, the erection tends to point lower after release, and some men notice a slightly less anchored feel at the base during intercourse.
Silicone girth implants (Penuma and Himplant class)
This is the only enlargement approach with a purpose-built, regulator-cleared device behind it. A crescent-shaped sheet of soft, medical-grade silicone is placed under the shaft skin, above the erectile bodies, through a single incision. The implant adds bulk around the shaft in both the flaccid and erect state. Surgery takes roughly 45-90 minutes under general anaesthesia, and uncircumcised men usually need circumcision first or at the same sitting.
The best available data come from a single-surgeon series of 400 patients: mean mid-shaft circumference, measured on the flaccid penis, increased by 56.7%, and 81% of men reported high or very high satisfaction at two to six years. Two caveats keep that honest. The study measured flaccid girth, and because the erect shaft starts from a larger baseline, the percentage gain when erect is meaningfully smaller than 56.7%. And the series reflects one highly experienced implanting surgeon rather than average results everywhere. Even so, it is stronger evidence than exists for fat, filler or graft techniques. One Bangkok-specific caveat: Penuma and Himplant are US-based device programmes delivered through a network of manufacturer-trained surgeons, and the genuine branded implant may not be available at any given Thai clinic. If a Bangkok surgeon offers "a Penuma-type implant", ask exactly which device will be used, by name, and verify its documentation with the manufacturer before paying. Two properties define this option. It is the most predictable way to add significant girth, and it is removable, although removal is its own procedure and can leave the penis temporarily or permanently changed, a point covered under risks.
Combined length and girth surgery
Some men have ligament release and a girth procedure in one operative plan, either simultaneously or staged a few months apart. Combining saves on anaesthesia and travel but lengthens recovery, and it only makes sense when a surgeon has examined you and agreed that both dimensions are genuinely treatable. Combined plans are where unrealistic expectations do the most financial damage, so this is the option that deserves the most sceptical questioning at consultation. Fat transfer and fillers, for context
Fat transfer moves your own fat into the shaft; hyaluronic acid filler injects a gel the body slowly absorbs. Both are cheaper than an implant and both are less predictable. Transferred fat partially resorbs over the first year, sometimes unevenly, and filler typically needs repeating every 12-24 months. They can be reasonable choices for men who want moderate girth without an implanted device, but they are maintenance treatments rather than one-time operations, and the lifetime cost can eventually overtake an implant. One firm warning: injections of liquid silicone, paraffin or unidentified "permanent filler" into the penis cause disfiguring granulomas and are never acceptable, at any price.
What actually drives the price
When two Bangkok quotes for the same procedure differ by 100,000 THB, the gap usually comes from a handful of variables:
The device. A genuine branded silicone implant has a fixed international cost that no clinic can discount away.
The surgeon. High-volume genital surgeons charge more and are worth it; revision surgery after a cheap first attempt is the most expensive path of all.
The setting. A full hospital theatre with a dedicated anaesthesia team costs more than an office procedure room. For implant surgery, the hospital-grade setting is not negotiable.
Add-ons. Circumcision, suprapubic fat removal or a scrotal web correction each add roughly 20,000-80,000 THB but are sometimes necessary for a clean result.
Aftercare structure. Packages that include traction devices, scheduled reviews and remote follow-up for international patients cost more upfront and less overall.
Admission. Day-case surgery versus one or two nights of observation.
Who is a good candidate, and who is not
Reasonable candidates share a few characteristics: they are adult men in stable general health, they can state a specific and modest goal, they understand the difference between flaccid and erect change, and they accept the recovery rules, including weeks of abstinence and, for lengthening, months of traction.
Surgery is a poor idea, and we will decline it, in several situations:
Body dysmorphic disorder or severe anxiety about a normal-sized penis. The evidence is consistent that counselling helps this group and surgery does not. Screening for this is part of our consultation, not an afterthought.
Expecting erect length gains. If your goal is a significantly longer erection, no honest surgeon can sell you that.
Uncontrolled diabetes, active skin or urinary infection, or a significant bleeding disorder. Each raises complication risk to unacceptable levels until treated.
Heavy smoking you will not pause. Nicotine impairs the skin healing these procedures depend on.
Unstable cardiovascular disease or any condition that makes elective general anaesthesia unwise.
Pressure from a partner. Surgery done to satisfy someone else predicts regret.
This is elective medicine done properly: a medical consultation, physical examination and surgeon assessment are required before any date or final price is confirmed, and a good clinic will sometimes conclude that the right treatment is no surgery at all.
From consultation to full recovery
The pathway at a well-run Bangkok clinic looks like this:
Consultation and examination. Measurement (flaccid, stretched and where relevant erect), health history, expectation screening and a frank discussion of which procedure, if any, fits your anatomy. Expect this to take 30-60 minutes.
Pre-operative work-up. Blood tests, anaesthesia review, and for implant candidates a plan covering circumcision status and skin quality. Usually completed within a week.
Surgery day. Ligament release takes about an hour; implant surgery 45-90 minutes. Most patients go home the same day or after one night of observation.
Days 1-7. Swelling, bruising and discomfort managed with oral pain relief. Desk work is usually possible within 3-5 days for ligament release and 5-7 days for an implant. Keep the wound dry as instructed.
Weeks 2-4. Sutures dissolve or are removed and swelling settles. Implant patients start to see the true contour emerge as the oedema resolves. No sexual activity yet, including masturbation.
Weeks 4-8. Gradual return to sexual activity with surgeon clearance, typically from week 4-6 after ligament release and week 6-8 after an implant. Lengthening patients are now well into the daily traction protocol.
Months 3-6. Final result assessment. For lengthening, traction usually continues to month 4-6; this is where the visible length gain is either consolidated or lost.

The results you can realistically expect
Numbers, with their sources and their caveats:
Ligament release: a mean gain of about 1.3 cm of stretched length in the published London series, extending towards 2 cm of visible flaccid length in men who complete a traction programme. Satisfaction in that series was only 35%, driven almost entirely by expectation mismatch. Men told the truth beforehand tend to be happier with the same physical result.
Silicone girth implant: a mean 56.7% increase in mid-shaft circumference in the 400-patient series, measured on the flaccid penis, which works out to roughly 4-6 cm of added flaccid girth for a typical starting size, with 81% high or very high satisfaction. The proportional gain when erect is smaller. Flaccid length appearance also improves modestly because the implant adds structure.
Fat transfer: girth gains of roughly 1-3 cm initially, of which a variable fraction, commonly quoted at 20-50%, resorbs during the first year.
Erect length, all techniques: essentially unchanged. If a clinic promises otherwise, keep your deposit.

Risks and side effects
Every genital operation carries risk, and the honest list is longer than most marketing pages admit.
Common and usually temporary: swelling, bruising, pain on erection during early healing, reduced or altered skin sensation for some weeks, minor wound-edge separation, and a visible scar at the incision.
Ligament release specific: a lower erection angle, a feeling of reduced stability at the base during intercourse, and re-attachment of the ligament with loss of the gained length, or occasionally net shortening, when traction is skipped.
Implant specific: seroma (a fluid collection), infection, palpable or visible implant edges, tilt or migration of the device, and skin erosion in thin tissue. An infected implant generally cannot be rescued with antibiotics alone; published management mirrors that of other penile prostheses, meaning surgical removal, and case reports document penile shortening and curvature after explantation. That is the true downside scenario to weigh before choosing an implant, and it is why surgeon selection and sterile hospital technique matter more than any discount.
Seek urgent care immediately, without waiting for a scheduled review, if you notice: fever above 38°C, spreading redness or heat around the wound, pus or foul-smelling discharge, rapidly increasing swelling or pain after the first week, inability to pass urine, or any darkening or dusky colour change of the shaft skin. Early treatment of infection is often the difference between saving and losing an implant.
For perspective: in experienced hands, most complications reported in the published series were minor and managed without losing the result, and only a small minority of implant patients required device removal. The complication profile worsens sharply with low-volume surgeons, non-hospital settings and unverifiable devices.
Choosing a safe surgeon in Bangkok
Bangkok has genuinely excellent genital surgeons and, like every medical-tourism hub, a tail of operators you should avoid. Work through this checklist before paying anyone:
Licence and specialty. A Thai Medical Council-licensed urologist or plastic surgeon who performs genital surgery regularly, not occasionally. Ask directly how many of this exact procedure they did last year.
The actual surgeon consults you. If you only ever speak to a sales coordinator before paying a deposit, walk away.
Hospital-grade theatre and a dedicated anaesthesia team for any implant work, with overnight observation available.
Device provenance. For a branded implant, ask to see the device documentation and verify it with the manufacturer, including whether that manufacturer supplies clinics in Thailand at all. A genuine device cannot be sold below its import cost.
A written, itemised quote that states what happens, and what it costs, if a revision is needed.
Expectation screening. Paradoxically, a clinic willing to refuse you surgery is the clinic you want operating on you.
