What tretinoin is & how it works
Tretinoin is a prescription topical retinoid, a vitamin A derivative (all-trans retinoic acid) applied to the skin to treat acne and the visible signs of sun damage, such as fine lines, rough texture and uneven pigment.
It works by changing how skin cells behave. Tretinoin binds receptors inside the cell that switch on genes controlling skin turnover. Old cells shed faster, pores stay clear instead of forming blackheads and spots, and over months the skin lays down new collagen. That is why the same molecule helps both breakouts and ageing skin.
It is not a quick cosmetic fix. Tretinoin retrains the skin over weeks and months, and the strength and routine that suit you depend on your skin type, your goals and how your skin reacts. That is what the doctor's assessment is for.

Getting tretinoin in Thailand
Thai FDA status
Prescription-strength topical tretinoin is a registered medicine in Thailand, sold under brands such as Retin-A. It is meant to be used under professional guidance, not bought casually off a shelf.⁴
How to get it through Menscape

Thai FDA warning. The regulator repeatedly warns against buying skin medicines from unlicensed online sellers and market stalls. Counterfeit, over-strength and mislabelled tretinoin is common, and nobody is accountable for what you receive.⁶

Does it work? The evidence
Tretinoin is the most-studied topical retinoid in dermatology, in clinical use since the 1970s. For acne, controlled trials show it reduces both clogged pores (comedones) and inflammatory spots over 8–12 weeks, and current guidelines recommend a topical retinoid as a first-line treatment for most acne.⁵
For sun-related ageing, the landmark evidence is a double-blind, vehicle-controlled trial (Weiss, JAMA 1988) in which tretinoin cream produced significant improvement in fine wrinkling, roughness and pigmentation, while the identical placebo cream did not. Two things to expect: results build over months, not weeks, and skin often looks worse before it looks better, with redness and flaking in the early weeks.

Side effects & who shouldn't use it
Common at first (the 'retinization' phase)
Redness, dryness, flaking, stinging and a temporary acne flare in the first 2–6 weeks as skin adjusts. Starting at a low strength and applying every other night makes this milder.⁵
Sun sensitivity
Tretinoin thins the outer skin layer and makes you burn more easily, and daylight breaks the molecule down. A daily broad-spectrum sunscreen is not optional with a retinoid, especially in Bangkok's sun.¹
Not suitable for
Pregnancy or breastfeeding, very sensitive or eczema- and rosacea-prone skin without supervision, and anyone with a known retinoid allergy. Tell your doctor if you are trying to conceive.
What not to combine
Avoid stacking it with strong exfoliating acids, waxing or harsh scrubs, and do not layer it with benzoyl peroxide at the same time of day. Your doctor will sequence products to limit irritation.

Frequently asked questions
Can I buy tretinoin over the counter in Thailand?
No. Prescription-strength tretinoin is a medicine, not a cosmetic, and should be used under a doctor's guidance. Some Bangkok pharmacies and online sellers offer it without oversight, but counterfeit and mislabelled products are common and the strength may be wrong for your skin.
How long until I see results?
Acne usually improves over 8–12 weeks, and sun-related ageing over 3–6 months, with the best results by 6–12 months. It is normal for skin to look redder and flakier, sometimes with a brief acne flare, in the first few weeks.
Why is my acne worse after starting tretinoin?
This is the 'retinization' or purging phase. Tretinoin speeds up cell turnover, so clogged pores surface faster before they clear. It typically settles within 4–6 weeks, and starting every other night makes it milder.
Can I use tretinoin if I am pregnant or trying to conceive?
Tretinoin should be avoided in pregnancy and breastfeeding. If you or your partner are trying to conceive, tell your doctor so they can suggest a safer alternative for your situation.
Does tretinoin really help wrinkles, or just acne?
Both. It is the most-studied topical for sun-related ageing: over months it stimulates collagen and evens pigment, softening fine lines and rough texture. Deeper wrinkles and sagging need other treatments.
Do I need sunscreen with tretinoin?
Yes, every day. Tretinoin makes skin more sun-sensitive and is broken down by daylight, so a broad-spectrum SPF is essential, especially in Bangkok. That is also why tretinoin is applied at night.
What strength should I start with?
That is a decision for the doctor, based on your skin type and goal. Most people start low, around 0.025%, and titrate up only if tolerated. Higher is not better if it leaves skin raw.
I already use tretinoin from abroad — can I continue it in Thailand?
Yes. A local doctor can review your current routine and prescribe an equivalent so you continue safely, rather than relying on grey-market refills of uncertain quality.
