What escin is & how it works
Escin is the active fraction of horse-chestnut seed extract (Aesculus hippocastanum), a mixture of natural saponins. It has been used in Europe and Asia for decades to reduce swelling and inflammation, and it comes in two forms: an oral tablet and a topical gel.
It works on the small blood vessels. When tissue is injured or operated on, tiny capillaries become leaky and fluid and blood escape into the surrounding area, which is what you see and feel as swelling and bruising. Escin lowers that capillary leakiness and raises the tone of the veins, so less fluid pools and trapped fluid drains away more easily. The effect is anti-edema and anti-inflammatory rather than pain-killing.
Escin is a supportive measure, not a cure and not a substitute for the basics of recovery. It does not speed up wound healing on its own, and it works alongside cold, compression and elevation rather than replacing them. Whether the tablet, the gel or both make sense for you depends on your procedure, injury or leg symptoms. That is what the pharmacist or doctor check is for.
Injury or surgery leaks fluid
Damaged small vessels leak fluid and blood into the surrounding tissue, which shows up as swelling and bruising.

It lowers the permeability of small blood vessels, so less fluid escapes into the tissue.¹

Escin is registered with the Thai FDA and sold as a pharmacy medicine, so no prescription is required. It is sold in Thailand under brands such as Reparil, in both an oral tablet and a topical gel.

Does it work? The evidence
The strongest evidence for escin is in chronic venous insufficiency: heavy, swollen, aching legs. A Cochrane systematic review of 17 randomised trials found that oral horse-chestnut seed extract, standardised to escin, reduced leg swelling, pain, itching and the feeling of heaviness compared with placebo.³ In one landmark 12-week trial it reduced leg volume about as much as wearing medical compression stockings.⁴
For post-operative and post-traumatic swelling and bruising, the evidence base is smaller and mostly older, but escin's anti-edema action is well characterised and it is widely used for this purpose. Treat it as supportive care: it works alongside cold, compression and elevation, not instead of them, and the benefit builds over days to weeks rather than instantly.

Side effects & who shouldn't take it
Common with the tablets
Nausea, stomach discomfort and occasionally dizziness or itching. Taking the tablets with food usually helps.⁵
With the gel
Mild skin redness or irritation is uncommon but possible. Do not apply the gel to broken skin, open wounds, fresh stitches or mucous membranes.
Serious but rare
Allergic reactions are rare. Stop and seek help for rash, facial swelling or trouble breathing. Note that an older injectable form of escin was linked to kidney problems; the oral tablet and gel used here are not injected.
Not suitable for
Not recommended in pregnancy or breastfeeding, for children, or with significant kidney impairment. Tell your doctor if you take blood thinners or antiplatelets, as escin may add a mild blood-thinning effect.
