What itraconazole is & how it works
Itraconazole is an oral triazole antifungal. It treats fungal infections that creams cannot reach: fungal nail infections (onychomycosis), widespread or stubborn skin fungus such as ringworm, jock itch and tinea versicolor, and certain deeper, systemic fungal infections.
It works by starving the fungus of a building block it depends on. Fungal cell membranes are held together by a fatty molecule called ergosterol. Itraconazole blocks a fungal enzyme that makes ergosterol, so the membrane becomes leaky and the fungus stops growing. Because the drug builds up in skin and nail keratin and lingers there for weeks, it keeps acting between doses, which is how short monthly pulses can clear a nail.
It is broader than terbinafine, covering yeasts and some moulds as well as the common dermatophytes. That is why a doctor may choose it for mixed or non-dermatophyte infections, and why the right choice depends on which fungus is actually present.
Fungus needs ergosterol
Fungal cell membranes depend on ergosterol to stay intact and grow.
Itraconazole blocks the enzyme
It inhibits lanosterol 14α-demethylase, the fungal enzyme that makes ergosterol.¹
The membrane fails
Without ergosterol the membrane turns leaky and the fungus stops multiplying.
It lingers in keratin
The drug concentrates in skin and nail and keeps working between pulses.¹

Getting itraconazole in Thailand
Thai FDA status
Registered with the Thai FDA and dispensed as a prescription-only medicine, classified among dangerous drugs (ยาอันตราย). It comes as capsules and an oral solution, sold in Thailand under brands such as Sporanox and registered generics.²
How to get it through Menscape

Safety vs the grey market
Do not buy antifungal 'nail cure' pills from unlicensed online sellers. Counterfeit and under-dosed products are common, and itraconazole's heart, liver and drug-interaction risks mean it needs medical oversight. No Thai residency is required to be seen; expats and visitors are welcome.

Does it work? The evidence
For fungal nail infections, itraconazole has decades of trial data. In toenail onychomycosis, pulse itraconazole achieves mycological cure in roughly 54–70% of people. Complete cure, meaning a fully clear nail plus negative tests, is lower, because a healthy nail must grow out first. In head-to-head trials, terbinafine tends to cure dermatophyte toenails at a higher rate, so the choice depends on the fungus involved.⁴
Two things are worth knowing. Nails clear slowly: a toenail can take 6–9 months to grow out even after the fungus is gone, so cure is judged then, not on the last day of dosing. And for serious systemic fungal infections such as talaromycosis, which is endemic in Thailand and South-East Asia, itraconazole is an established treatment used under specialist supervision.⁵

Side effects & who shouldn't take it
Common side effects
Nausea, stomach upset, diarrhoea, headache and, less often, a temporary rash. Most are mild. Taking the capsule with a full meal improves both absorption and tolerability.
Serious but rare
Liver injury, rarely severe, can occur: report dark urine, yellowing skin or eyes, or persistent nausea. Because itraconazole can weaken the heart's pumping, new or worsening breathlessness or ankle swelling should be checked promptly.
Who shouldn't take it
Anyone with congestive heart failure or a history of it must not use it for nail or skin fungus (boxed warning). Also avoid in significant liver disease and in pregnancy, and tell your doctor if you are trying to conceive.
Interactions & warnings
It strongly blocks CYP3A4, so it is dangerous with certain statins (for example simvastatin, lovastatin), some heart-rhythm drugs, ergot medicines and certain sedatives. Antacids and acid-reducing tablets cut absorption of the capsule. Bring a full list of your medicines and supplements.¹

Oral · often first-line
Terbinafine
For common dermatophyte nail and skin infections, terbinafine is often first-line, with high cure rates and fewer drug interactions. A doctor may prefer it unless a yeast or mould is involved.
Topical · mild or early cases
Topical antifungals
Creams and medicated nail lacquers such as amorolfine or efinaconazole suit mild, early nail infection or limited skin fungus, without the systemic risks. Slower to work, but gentler.
Oral · alternative azole
Fluconazole
Another triazole, useful for yeast infections and some skin fungus. It is weaker against dermatophyte nails but has a different interaction profile a doctor may prefer in some cases.

Frequently asked questions
Can I buy itraconazole over the counter in Thailand?
No. It is a prescription-only medicine, classified among dangerous drugs (ยาอันตราย), and must come from a pharmacist or doctor. Because it can affect the heart and liver and interacts with many drugs, self-treating with pills bought online is risky and often counterfeit.
How long is a course of itraconazole for nail fungus?
For toenails, a common approach is pulse dosing: a one-week course each month, repeated for about three months (two for fingernails). Continuous daily courses are also used. Your doctor sets the exact regimen based on the fungus and your nails.
Why does my nail still look bad after I finish the pills?
That is normal. The medicine clears the fungus, but the damaged nail has to grow out and be replaced by healthy nail, which takes about 6–9 months for a toenail. Cure is judged then, not on the last day of dosing.
Is itraconazole safe for my heart?
It carries a boxed warning: it must not be used for nail or skin fungus in anyone with congestive heart failure or a history of it, because it can weaken the heart's pumping. Tell your doctor about any heart condition. This is one reason a consultation comes first.
Can I keep taking my other medicines while on it?
Not without a review. Itraconazole interacts with many drugs, including some statins, heart-rhythm medicines and sedatives, so never combine it on your own. Heavy alcohol adds to liver strain. Bring a complete list of everything you take, including supplements.
Should I take it with food?
The capsule is absorbed much better with a full meal, and antacids or acid-reducing tablets can block its absorption. Follow the exact instructions your doctor and pharmacist give, because the oral solution is taken differently from the capsule.
Itraconazole or terbinafine: which is better for nail fungus?
For common dermatophyte toenail infections, terbinafine often cures at a higher rate with fewer interactions. Itraconazole is broader and may be chosen for yeasts, moulds or mixed infections, or when terbinafine is not suitable. Testing the nail helps decide.
I already take itraconazole abroad. Can I continue it in Thailand?
Yes. A local doctor can review your treatment and, if appropriate, arrange a prescription. Bring your diagnosis, any test results and your current dose. No Thai residency is required.
