What isosorbide dinitrate is & how it works

Isosorbide dinitrate is a prescription nitrate used to prevent and relieve angina, the chest pain or tightness that happens when the heart muscle is not getting enough blood. It is also used, in combination with other medicines, in some cases of heart failure.

It works by widening blood vessels. In the vessel wall, the nitrate is released as nitric oxide, the body's own signal to relax smooth muscle. Veins widen first, which cuts the amount of blood returning to the heart (the preload), so the heart has less work to do and needs less oxygen. Coronary arteries also dilate, improving blood flow to the heart itself. The result is fewer and less severe anginal episodes.

That same nitric-oxide pathway is why nitrates and ED tablets are a dangerous mix. PDE5 inhibitors such as sildenafil and tadalafil act on the very next step in the same chain. Taken together, the two stack up and can cause a sudden, severe drop in blood pressure. This is not a mild interaction. It is a hard contraindication, and it is the single most important thing to understand about this medicine.

    In the blood-vessel wall, isosorbide dinitrate is converted to nitric oxide.¹

    Nitric oxide triggers a messenger called cGMP inside vascular smooth muscle.

  3. Vessels relax

    Veins widen to reduce preload; coronary arteries dilate to improve flow.¹

  4. The heart eases

    With less workload and better supply, angina is prevented or relieved.

Getting isosorbide dinitrate in Thailand

Thai FDA status

Registered with the Thai FDA and dispensed as a prescription-controlled cardiac medicine (ยาควบคุมพิเศษ). Sold in Thailand under brands such as Isordil, alongside registered generics.² It is not a product to self-select from a shelf.

Does it work? The evidence

Nitrates are among the oldest and best-established anti-anginal medicines, in clinical use for more than 60 years. Sublingual isosorbide dinitrate relieves an acute anginal attack within minutes, and regular dosing reduces how often angina strikes and how far a person can exert themselves before it does. Major cardiology guidelines list nitrates as core symptom-control therapy for angina.⁴ ⁵

Two honest points before you start. First, nitrates treat symptoms: they have not been shown to reduce heart attacks or deaths in stable angina, so they sit alongside, not instead of, treatments that do. Second, taken around the clock they lose effect (tolerance), which is why a daily nitrate-free interval of 10–14 hours is built into the dosing.¹

Side effects & who shouldn't take it

Common side effects

Headache is the most common, especially in the first days, along with flushing, dizziness, a fast heartbeat, and lightheadedness on standing from lowered blood pressure. These often ease with time; do not stop suddenly.¹

Serious but less common

A sharp fall in blood pressure can cause fainting, particularly with the first dose, alcohol, or dehydration. Very high doses can rarely cause methaemoglobinaemia. Sit or lie down if you feel faint, and seek care if it persists.

Not suitable for

Anyone taking a PDE5 inhibitor (sildenafil, tadalafil, vardenafil) or riociguat, and people with very low blood pressure, hypertrophic obstructive cardiomyopathy, severe aortic stenosis, or a recent right-sided heart attack. Tell your doctor your full history.

The interaction that matters most

Never take an ED tablet while you use a nitrate, and never take a nitrate within 24 hours of sildenafil or vardenafil, or 48 hours of tadalafil. The combination can be fatal. Alcohol and other blood-pressure medicines also add to the effect.⁶

Glyceryl trinitrate (GTN)

A fast-acting nitrate spray or tablet used under the tongue for breakthrough angina. It carries the exact same ED-pill contraindication as isosorbide dinitrate.

Prevention · first-line

Beta-blockers & calcium channel blockers

Guideline first-line options for preventing angina and, unlike nitrates, some also improve outcomes. Nitrates are often added on top for extra symptom control.

Can I buy isosorbide dinitrate over the counter in Thailand?

No. It is a prescription-controlled cardiac medicine that a doctor must prescribe after confirming the diagnosis. Self-treating chest pain is dangerous. It can be a warning sign of something serious that needs proper assessment.

Can I take an ED pill like sildenafil or tadalafil if I use a nitrate?

No. Combining a nitrate with a PDE5 inhibitor can cause a sudden, severe and potentially fatal drop in blood pressure. This is a firm contraindication, not a caution. Always tell any doctor prescribing ED medication that you take a nitrate.

I took an ED pill and now have chest pain — can I use my nitrate?

Do not take a nitrate within 24 hours of sildenafil or vardenafil, or 48 hours of tadalafil. Call emergency services (1669 in Thailand) and tell them exactly what you took and when. This is the situation the whole nitrate–PDE5 rule exists to prevent.

Why does isosorbide dinitrate give me a headache?

Nitrate headache comes from the same vessel-widening that helps your heart, and it is very common at first. It usually settles within a few days as your body adjusts. Do not stop the medicine on your own; speak to your doctor if it is severe.

What is a nitrate-free interval and why does it matter?

Taken around the clock, nitrates lose their effect within a day or two (tolerance). Leaving a daily gap of about 10–14 hours with no nitrate in your system keeps the medicine working. Your doctor will build this gap into your dosing schedule.

Does isosorbide dinitrate treat erectile dysfunction?

No. It is a heart medicine for angina, not a treatment for ED, and it actually rules out the standard ED tablets because of the dangerous interaction. If you have both angina and ED, a doctor can plan a safe approach for you.

Can I stop taking it suddenly?

No. Stopping a nitrate abruptly can trigger rebound angina or coronary spasm. Any change should be planned and tapered with your doctor rather than done on your own.

I already take a nitrate from abroad — can I continue it in Thailand?

Usually yes. Bring your medication details and recent records, and a local doctor can review your treatment and provide a prescription. Continuity of care matters most with heart medicines, so do not simply run out.
